Advocare participates with most major insurance plans. As participation varies by region, specialty, and physician, we recommend that you contact your insurance company representative to verify that your insurance plan is accepted.
Please have your insurance card with you at the time of your visit. If we file your claim, you are responsible for paying the co-pay and any previous balance at the time of visit.
Although we have agreed to serve as providers for many insurance plans, the terms of the policies vary, even within a particular insurance program. Please call your insurance company so you can understand exactly what your insurance covers and especially the requirements specific to your policy (office co-pays, deductibles, Telemedicine, referrals to specialists, eligible hospitals, emergency care, etc.)
